COVID-19 Protocols

 

1. You agree to not come to your appointment location if you have been diagnosed with COVID-19.

2. You agree to not come to your appointment location if you have symptoms of COVID-19.

​

3. You agree to not come to your appointment location if you have symptoms of COVID-19, but not limited to: fever, chills, repeated shaking with chills, muscle pain/aches, headache, sore throat, loss of taste or smell. 

​

4. You agree to practice good personal hygiene: washing hands for 20+ seconds with warm soapy water, covering mouth and nose when coughing and/or sneezing, using hand sanitizer, wearing a face covering at all times on the lot location. 

​

5. You agree to practice social distancing guidelines (i.e 6 ft or 2 meters away from other individuals).

​

6. You agree to not come to your appointment location if you have traveled outside your home state/providence.
